Is Osteoarthritis the Cause of Your Hip Pain !!


Arthritis of the hip is a disease which wears away the cartilage between the femoral head and the acetabulum, the two bones will scrape against each other, raw bone on raw bone. When this happens, the joint becomes pitted, eroded and uneven. The result is pain, stiffness and instability. In some cases, motion of the leg may be greatly restricted.

Osteoarthritis

Osteoarthritis is the most common form of arthritis. It is degenerative and although it most often occurs in patients over the age of 50, it can occur at any age, especially if the joint is in some way damaged.





It is usually confined to the large weight-bearing joints of the lower extremities, including the hips and knees, but may affect the spine and upper extremity joints, too. Patients with osteoarthritis often develop large bones spurs, or osteophytes, around the joint, further limiting motion.

Causes

Osteoarthritis of the hip is a condition commonly referred to as "wear and tear" arthritis. Although the degenerative process may accelerate in persons with a previous hip injury, many cases of osteoarthritis occur when the hip simply wears out. Some experts believe there may exist a genetic predisposition in people who develop osteoarthritis of the hip. Abnormalities of the hip due to previous fractures or childhood disorders may also lead to a degenerative hip. Osteoarthitis of the hip is the most common cause for total hip replacement surgery.

Symptoms


The first and most common symptom of osteoarthritis is pain in the lip or groin area during weight bearing activities such as walking. People with hip pain usually compensate by limping, or reducing the force on the arthritic hip. As a result of the cartilage degeneration, the hip loses its flexibility and strength, and may result in the formation of bone spurs.
Finally, as the condition worsens, the pain may be present all the time, even during non weight-bearing activities.

Treatment


Before considering total hip replacement surgery, your doctor and you may try various non-surgical therapies. An appropriate weight reduction program may be beneficial in decreasing force across the hip joint. However, weight reduction can be difficult for people with hip arthritis since the arthritis pain precludes them from increasing their activity and burning calories. An exercise program may be instituted to improve the strength and flexibility of the hip and the other lower extrremity joints. Lifestyle and activity modification may be undertaken in an attempt to minimize the activities that are associated with hip pain. Finally, various medications such as non-steroidal anti-inflammatory drugs and nutritional supplements to reduce pain and inflammation associated with the disease may be considered.

Assistive devices like a cane or a crutch can help to reduce the force transmitted through the hip joint during walking and thereby may help to decrease hip arthritis pain. If non-surgical treatment is unsuccessful, you and your surgeon may decide that a total hip replacement is the best available treatment option.

Best Solution for Shoulder Impingement / Rotator Cuff

One of the most common physical complaints is shoulder pain. Your shoulder is made up of several joints combined with tendons and muscles that allow a great range of motion in your arm. Because so many different structures make up the shoulder, it is vulnerable to many different problems. The rotator cuff is a frequent source of pain in the shoulder.

The shoulder impingement or rotator cuff is a common source of pain in the shoulder. Pain can be the result of:
  • Tendinitis. The rotator cuff tendons can be irritated or damaged.
  • Bursitis. The bursa can become inflamed and swell with more fluid causing pain.
  • Impingement. When you raise your arm to shoulder height, the space between the acromion and rotator cuff narrows. The acromion can rub against (or "impinge" on) the tendon and the bursa, causing irritation and pain.

Cause
Rotator cuff pain is common in both young athletes and middle-aged people. Young athletes who use their arms overhead for swimming, baseball, and tennis are particularly vulnerable. Those who do repetitive lifting or overhead activities using the arm, such as paper hanging, construction, or painting are also susceptible.

Pain may also develop as the result of a minor injury. Sometimes, it occurs with no apparent cause.

Symptoms
Rotator cuff pain commonly causes local swelling and tenderness in the front of the shoulder. You may have pain and stiffness when you lift your arm. There may also be pain when the arm is lowered from an elevated position.

Beginning symptoms may be mild. Patients frequently do not seek treatment at an early stage. These symptoms may include:

  • Minor pain that is present both with activity and at rest
  • Pain radiating from the front of the shoulder to the side of the arm
  • Sudden pain with lifting and reaching movements
  • Athletes in overhead sports may have pain when throwing or serving a tennis ball
As the problem progresses, the symptoms increase:

  • Pain at night
  • Loss of strength and motion
  • Difficulty doing activities that place the arm behind the back, such as buttoning or zippering
If the pain comes on suddenly, the shoulder may be severely tender. All movement may be limited and painful.

Joint preservation concerns with the restoration of the normal function of the joint, may be it knee, hip or the shoulder. The technique is an emerging as a better option for joint replacement procedure. The procedure is reliably employed as a way to prevent or delay the onset of osteoarthritis, the most common form of arthritis. Relatively younger patients are seen to take benefit from this mode of treatment.


Couple of years ago, joint replacement was the procedure that captured the attention. Many patients successfully tried this procedure. Majority of them was middle aged. The pelvic ring stability is thus preserved. Prior to the surgery, a preoperative planning of PAO is done. The planning aims to make an appropriate pelvis model. With the help of this model, the reorientation of the hip socket is planned.

The advantages of PAO are many. The primary plus point is a much larger weight bearing ability of the hip. Females who undergo this surgery can have normal vaginal deliveries rather going in for the cesarean delivery.

The rehabilitation period is almost 6-8 weeks and is continued until some recovery signs in the osteotomy site are seen. Weight bearing can be increased slightly. Nearly every patient will be able to walk after a period of 3-4 months.
